Cause and Origin Determination

In the devastating situation of an accident, incident, or failure the most important questions are “Where?” and “How?” But we must also determine the Who, What, and When. To do this, Packer investigators pinpoint exactly where the accident, incident or failure originated by using the “scientific method” which is a process of inquiry relying on four elements: observation, hypothesis, theory, and experiment. The underlying objective is always the same even though interaction between these four elements may differ from one situation to another.  Proper cause and origin determination is vital to understanding why it occurred and how it can be prevented in the future.
